Description
Arising from the postauricular scalp, the posterior auricular vein courses inferiorly and slightly anteriorly in the groove posterior to the auricle, running superficial to the mastoid process and paralleling the posterior auricular artery. From this lateral head and neck perspective in a male, the vein sits posterior to the external acoustic meatus and lateral to the upper sternocleidomastoid, with the auricularis posterior and adjacent occipitalis region forming key soft tissue landmarks. Branches of the facial nerve (CN VII) fan across the parotidomasseteric region anterior to the ear, while the greater auricular nerve (C2-C3) tracks superiorly over the sternocleidomastoid toward the lobule and parotid fascia. Small venules from the posterior auricular and mastoid territories converge toward the main trunk. This angle matters because the posterior auricular vein is a practical landmark for the retroauricular corridor, where skin, fascia, and neurovascular structures crowd the mastoid region in a small working space. Surgeons use this territory for mastoidectomy and postauricular incisions in otologic procedures, and accidental venous injury can obscure the field quickly. The illustration also supports teaching venous drainage patterns, including typical continuity with the external jugular system via the posterior division of the retromandibular vein, and common anatomic variation in the superficial venous network around the ear.
