Description
Running along the inferior margin of each rib, the posterior intercostal veins trace a segmental ladder within the costal grooves, positioned deep to the intercostal musculature and closely paired with the intercostal arteries and nerves in the neurovascular plane. From a lateral thoracic profile, the ribs and costal cartilages frame the vessels as they course posteriorly toward the vertebral column, where they approach the heads and necks of the ribs adjacent to the thoracic vertebrae. Superior veins lie progressively higher and more posterior, while inferior veins descend toward the lower intercostal spaces. Orientation is clean. Landmarks read immediately. Clinically, this view helps clarify where blood from the thoracic wall drains into the azygos system on the right and the hemiazygos and accessory hemiazygos pathways on the left, with known anatomical variation by level that matters when correlating venous anatomy to CT angiography or posterior mediastinal surgery. Segmental veins also sit in harm’s way during chest tube placement and thoracentesis, where a needle or trocar placed too close to the inferior rib border can lacerate the intercostal vessels. That complication is preventable. The lateral perspective makes the safe corridor above the rib easy to teach.
