Description

Running along the lateral compartment of the leg, the common fibular (peroneal) nerve is seen dividing near the fibular neck into the superficial fibular nerve and the deep fibular (deep peroneal) nerve. From this lateral perspective, the deep fibular nerve tracks anteriorly to the fibula and then descends in close company with the anterior tibial vessels along the interosseous membrane, deep to tibialis anterior and extensor digitorum longus. Distally it approaches the ankle joint and dorsal foot, where its terminal branches course toward the first web space between the hallux and second toe. Orientation is clean. Bones of the femur, tibia, fibula, and the tarsals provide fixed landmarks for the nerve’s path. This angle matters because it clarifies the relationship between the deep fibular nerve and the fibular head, a site where traction or compression can follow knee dislocation, fibular neck fracture, or tight plaster casting, producing foot drop from weakness of ankle dorsiflexion. The descending course adjacent to the interosseous membrane also helps explain why anterior compartment syndrome can compromise deep fibular nerve function and cause sensory loss in the dorsal first web space.
