Description
Medial exposure of the male lower limb tracks the sciatic nerve distally as it descends posterior to the femur and approaches the popliteal fossa, where it separates into the tibial nerve and the common fibular (peroneal) nerve. The common fibular nerve then courses laterally along the proximal calf, skirting the biceps femoris tendon region toward the fibular head and neck, while the tibial nerve remains more medial and deep, continuing into the posterior compartment. Bony landmarks anchor the pathway, including the pelvis superiorly, the femur and patella at the knee, and the tibia and fibula extending to the tarsals, metatarsals, and phalanges. Nerves are rendered in yellow against the skeletal framework for clear spatial orientation. Understanding where the common peroneal nerve leaves the sciatic trunk matters because its superficial course around the fibular neck makes it one of the most frequently injured major peripheral nerves in the lower extremity. This relationship explains classic foot drop after fibular neck fracture, tight casting, prolonged leg crossing, or lateral knee trauma, and it frames the sensory loss over the dorsum of the foot when the superficial fibular branch is involved.
