Description
Running anterior to the fibula after its origin from the common fibular (peroneal) nerve, the deep fibular nerve is traced from the lateral aspect of the popliteal region across the fibular neck and into the anterior compartment of the leg. From there it descends on the interosseous membrane with the anterior tibial vessels, lying between tibialis anterior medially and extensor digitorum longus laterally, then continuing toward the dorsum of the foot. Bony landmarks frame the course, including the distal femur, patella, tibial crest, fibular shaft, and the tarsal and metatarsal bones distally. Blue venous channels accompany the neurovascular pathway, emphasizing the layered relationships from knee to ankle. Clinically, this anterior perspective is the one you want when teaching or planning around deep fibular neuropathy and anterior compartment syndrome, where rising compartment pressure can compromise the nerve and anterior tibial artery together. Pain or paresthesia in the first dorsal web space and weakness of toe extension map cleanly to the distal deep fibular nerve as it passes beneath the extensor retinacula, a tight tunnel prone to entrapment in ski boot or lace bite injuries. The proximal segment at the fibular neck also highlights why lateral knee trauma, tight casts, or fibular head fractures can injure the fibular nerve before it divides.
