Description
Seen in lateral profile, the proximal ulna is rendered with emphasis on the coronoid process and the trochlear notch, where the anterior lip has fragmented into multiple shards. The olecranon lies posterior and superior to the notch, forming the posterior buttress of the elbow, while the ulnar shaft continues distally in near anatomic alignment. Fracture lines propagate through the coronoid base, disrupting the smooth concavity that normally articulates with the humeral trochlea. A small detail with big consequences. Coronoid fractures are less about bone pain and more about elbow stability. Loss of the anterior ulnohumeral buttress permits posterior subluxation or frank dislocation, classically in the terrible triad pattern (elbow dislocation with radial head fracture and lateral ulnar collateral ligament injury), and even small anteromedial facet fragments can drive varus posteromedial rotatory instability when the lateral collateral complex fails. This lateral perspective supports teaching of why CT-based fragment characterization and fixation strategy matter, from suture lasso or screw fixation of a tip fracture to buttress plating when the fracture involves the anteromedial facet or base. Orthopaedic trauma courses and elbow surgery chapters use this type of view to explain coronoid morphology, O’Driscoll-type fracture patterns, and the rationale for restoring the ulnohumeral articulation before addressing ligament repair.
