Description
Running longitudinally along the posterolateral neck, the iliocostalis cervicis appears as the lateral column of the erector spinae complex, positioned lateral to longissimus cervicis and posterior to the transverse processes of the mid to lower cervical vertebrae. Its superior fibers course toward the posterior tubercles of the cervical transverse processes (classically C4 to C6), while the inferior portion blends with the iliocostalis thoracis as it approaches the upper ribs. A lateral male view also logically includes the cervical vertebral bodies and posterior elements, the upper thoracic ribs, and the scapula and clavicle framing the shoulder girdle. Orientation is clear. The muscle sits deep to trapezius and adjacent to levator scapulae near the superior angle of the scapula. This perspective matters when teaching why posterior neck pain can localize lateral to the spinous processes and worsen with ipsilateral lateral flexion and extension. Cervical facet-mediated pain and myofascial trigger points in the erector spinae often refer to the occiput or shoulder region, and the iliocostalis cervicis is a frequent contributor that gets lumped under the broader label of paraspinals. For interventional planning, the lateral relationship between the transverse processes and the overlying muscle mass helps contextualize fluoroscopic or ultrasound approaches for cervical medial branch blocks and radiofrequency ablation, where depth and lateral offset drive needle trajectory.
