Description
Running as a longitudinal column along the posterior cervical and upper thoracic spine, the iliocostalis cervicis is positioned lateral to the spinous processes and medial to the posterior border of the scapula. Superiorly it approaches the transverse processes of the mid cervical vertebrae (typically C4 to C6), while inferiorly it blends into the iliocostalis thoracis on the ribs and upper thoracic region as part of the erector spinae complex. A posterior male view also logically includes the occiput and cervical vertebrae, upper thoracic vertebrae, ribs, and both scapulae, with the levator scapulae running superomedial from the superior angle of the scapula toward C1 to C4, superficially adjacent to the cervical extensors. Relationships matter here: iliocostalis cervicis sits lateral to spinalis and longissimus, and deep to superficial layers such as trapezius when those are present. Clinically, this segment of the erector spinae is a frequent generator of paraspinal myofascial pain in the neck and interscapular region, often reported after sustained forward head posture and prolonged computer work. Palpation and dry needling approaches rely on recognizing its lateral position relative to the cervical spinous processes and its proximity to the levator scapulae, a common co-contributor in scapular elevation and neck rotation complaints. Small errors in localization can shift attention to longissimus cervicis or the levator scapulae. That changes the exam.
