Description
Positioned as the lowest muscular layer of the pharyngeal wall, the inferior pharyngeal constrictor forms a broad, funnel-shaped sheet wrapping posterolaterally around the hypopharynx in a male neck. Its fibers arise anteriorly from the thyroid cartilage (thyropharyngeus) and the cricoid cartilage (cricopharyngeus), then sweep posteriorly to meet at the median pharyngeal raphe. Superiorly it overlaps the middle pharyngeal constrictor, while inferiorly the cricopharyngeal part blends into the upper esophageal sphincter at the pharyngoesophageal junction. Deep and anterior to it, the laryngeal framework is apparent, with thyroid and cricoid cartilages providing key landmarks for orientation. Functionally, this muscle matters because the cricopharyngeus segment is the principal contractile component of the upper esophageal sphincter, the gatekeeper between hypopharynx and cervical esophagus. Dyscoordination or hypertonicity here underlies cricopharyngeal dysfunction and contributes to Zenker diverticulum, which forms through Killian dehiscence just superior to the cricopharyngeus between the thyropharyngeus and cricopharyngeus fiber directions. Surgeons and speech language pathologists also track its relationship to the recurrent laryngeal nerve and the posterior laryngeal inlet when planning endoscopic dilation, botulinum toxin injection, or cricopharyngeal myotomy.
