Description
Arising from the greater and lesser cornua of the hyoid bone and from the stylohyoid ligament, the middle pharyngeal constrictor forms a broad sheet that fans posteriorly to the midline pharyngeal raphe. Superiorly it overlaps the inferior margin of the superior pharyngeal constrictor, while its inferior fibers are overlapped by the superior edge of the inferior pharyngeal constrictor, creating the layered muscular wall of the male pharynx. From this posterior perspective, the constrictor complex sits deep to the prevertebral fascia and anterior to the cervical vertebral column, with adjacent carotid system vessels coursing laterally in the neck. Lymphatic channels and nodes track along expected jugular chain pathways. Posterior visualization of the middle constrictor matters because it clarifies where bolus propulsion is generated at the level of the hyoid and how the constrictor laminae interdigitate at their borders, a common point of confusion in teaching. The relationship to lateral vascular structures and regional lymphatics is also clinically relevant when discussing transoral and transcervical approaches to the pharynx, as well as pathways of spread for oropharyngeal malignancy to the deep cervical lymph nodes.
