Centered at the pharyngoesophageal junction, the cricopharyngeus forms the transverse, lower portion of the inferior pharyngeal constrictor, wrapping posteriorly and laterally around the upper esophageal sphincter at approximately the C5 to C6 level. Superior to it lie the thyropharyngeus fibers, while inferiorly the muscular wall transitions into the cervical esophagus; the posterior midline raphe of the pharynx sits just dorsal to the laryngeal framework and trachea. Laterally, the expected companions in this male neck view include the sternocleidomastoid and scalene muscle mass, with the carotid sheath contents, common carotid artery and internal jugular vein, positioned anterolateral to the pharynx. Regional lymphatics track along the internal jugular chain. A tight corridor. Cricopharyngeus anatomy matters because it is the functional gatekeeper of swallowing, and its failure to relax produces cricopharyngeal dysfunction with dysphagia, pharyngeal residue, and aspiration risk on videofluoroscopic swallow study. Hypertrophy or discoordination at this level contributes to a Zenker diverticulum by raising hypopharyngeal pressure and forcing mucosa through Killian dehiscence, just superior to the cricopharyngeal fibers.
