Seen from the lateral aspect of an adult male trunk, the internal oblique (musculus obliquus internus abdominis) lies deep to the external oblique and superficial to transversus abdominis, its fibers coursing superomedially toward the inferior borders of ribs 10 to 12 and the linea alba. Superiorly, the muscle contributes to the costal margin and blends with the thoracolumbar fascia posteriorly, while inferiorly it arises from the iliac crest and inguinal ligament and forms a layered abdominal wall with the adjacent aponeuroses. Medially, its aponeurosis participates in the rectus sheath; laterally, it transitions into muscular slips over the flank. Layering is the point. For teaching and procedural planning, a lateral view clarifies how the internal oblique and transversus abdominis create the neurovascular plane that carries the intercostal nerves and vessels, and how that relationship guides trunk incisions. This is the fascial and muscular corridor exploited in transversus abdominis plane (TAP) blocks, where anesthetic is placed between internal oblique and transversus to provide analgesia after lower abdominal surgery. Surgeons also use this anatomy in flank approaches and during hernia repairs to avoid denervating the abdominal wall and to preserve the integrity of the inguinal region. Common uses include abdominal wall anatomy lectures, anesthesia education modules on TAP and ilioinguinal/iliohypogastric blocks, and surgical atlases covering ventral and inguinal hernia repair, appendectomy access, and donor site planning for abdominal-based flaps.
