Description
Running inferomedially from the lateral thoracoabdominal wall, the internal oblique muscle (obliquus internus abdominis) occupies the layer deep to external oblique and superficial to transversus abdominis. Its fleshy fibers arise from the thoracolumbar fascia, anterior two thirds of the iliac crest, and lateral inguinal ligament, then sweep superiorly and medially toward the costal margin and linea alba. Medially, the internal oblique contributes an aponeurosis to the rectus sheath, forming an anterior lamina in the upper abdomen and, below the arcuate line, passing entirely anterior to rectus abdominis to meet the linea alba. Ribs, costal cartilages, and the superior pelvis provide bony reference for this relationship. Medial tendon sheet anatomy matters because the internal oblique is a key contributor to the rectus sheath and to the inguinal canal roof through its arching fibers. Get this layer wrong and you misread the arcuate line. In open ventral hernia repair, Pfannenstiel and paramedian incisions, or component separation planning, surgeons rely on the transition from posterior rectus sheath to transversalis fascia, and on how internal oblique aponeurosis distributes tension across the anterior abdominal wall. It also frames why weakness in the conjoint tendon region can predispose to direct inguinal hernia.
