Description
Running anterior to the auricle and draped over the ramus of the mandible, the parotid lymphatics are shown as superficial channels within the parotid fascia and deeper collectors embedded in the substance of the parotid gland. Along the posterolateral face, lymphatic pathways track close to the parotid duct as it courses over the masseter toward the buccinator, while deeper routes lie adjacent to the retromandibular vein and the external carotid artery within the gland. Branches of the facial nerve fan laterally across the gland, crossing the lymphatic field between the zygomatic arch superiorly and the angle of the mandible inferiorly. The sternocleidomastoid and posterior belly of the digastric frame the posterior and inferior margins, orienting the drainage corridor toward upper deep cervical nodes. Surgeons care about this lateral anatomy because parotid lymphatic drainage governs both the pattern of regional metastasis and the logic of nodal sampling for cutaneous squamous cell carcinoma and melanoma of the temple, lateral scalp, and external ear. Fine relationships matter: lymph nodes can sit superficial, intraparotid, or deep to the facial nerve branches, so a parotidectomy for intraparotid nodal disease can place the temporofacial and cervicofacial divisions at risk. Facial weakness is the complication everyone remembers.
