Description
Seen from the lateral pelvis of an adult male, the sacrotuberous ligament runs inferolaterally from the posterolateral sacrum and coccyx toward the ischial tuberosity, forming a dense band along the posterior margin of the hip. Posterior to the ischium, it lies deep to the gluteus maximus and adjacent to the proximal hamstring origin at the ischial tuberosity, while the greater sciatic foramen sits superior to its sacral attachment and the lesser sciatic foramen is bounded just anterior to it. Bony landmarks typically visible in this angle include the posterior ilium, acetabulum, proximal femur, and the ischial spine as the sacrospinous ligament courses toward it. Clear spacing matters here. It defines foraminal anatomy. Clinically, this lateral relationship is the one you want when teaching how the sacrotuberous and sacrospinous ligaments convert the greater and lesser sciatic notches into foramina and guide neurovascular corridors exiting the pelvis. The pudendal nerve and internal pudendal vessels wrap around the ischial spine and pass between these ligaments into the perineum, a pathway discussed in pudendal neuralgia and during landmark-based pudendal nerve blocks. Pelvic ring mechanics also live here: tensioning of the sacrotuberous ligament resists counternutation of the sacrum and can be implicated in posterior pelvic pain patterns, while its proximity to the hamstring origin makes it a useful reference when differentiating proximal hamstring tendinopathy from deep gluteal pain.
