Description
Seen from a lateral angle, the second thoracic vertebra (T2) presents a heart-shaped vertebral body anteriorly with the vertebral arch sweeping posteriorly to form the vertebral foramen, then continuing into the spinous process, which angles inferiorly compared with cervical levels. The pedicle bridges the body to the lamina, and the transverse process projects posterolaterally from the junction of pedicle and lamina, creating a clear relationship between the vertebral body, transverse process, and posterior elements. Along the superior and inferior margins of the body, the costal demifacets for the head of the rib sit at the posterolateral corners, while the transverse costal facet on the transverse process marks the synovial articulation with the tubercle of rib 2. This lateral presentation matters because T2 is a transition point between the more mobile cervical spine and the rib-stiffened thoracic spine, and its rib articulations often explain patient pain patterns that seem “spinal” but originate at the costovertebral or costotransverse joints. Small osteophytes or inflammatory change at these joints can restrict thoracic rotation and contribute to posterior shoulder girdle discomfort, and they are easy to miss without a mental map of where the transverse process sits relative to the vertebral body. It also orients planning for posterior thoracic approaches, where the lamina, facet joints, and transverse process guide safe trajectories around the spinal canal.
