Seen in lateral profile, the thyrohyoideus spans from the oblique line of the thyroid cartilage superiorly to the inferior border of the hyoid bone, sitting deep to where the sternohyoid and omohyoid would lie in the anterior cervical compartment. The superior horn and lamina of the thyroid cartilage form the dominant anterior landmark, with the cricoid cartilage positioned inferior and slightly posterior, completing the laryngeal framework. Intrinsic laryngeal muscles such as the cricothyroid on the anterolateral cricoid and thyroid, and the thyroarytenoid within the thyroid lamina, contextualize the thyrohyoideus as an infrahyoid strap muscle closely apposed to the laryngeal cartilages. Small but telling. Functionally, the thyrohyoideus is the strap muscle most directly coupled to laryngeal elevation because it can approximate the thyroid cartilage to the hyoid, or depress the hyoid when the larynx is fixed, depending on which structure is stabilized by surrounding suprahyoid and infrahyoid tone.
