Description
Arising from the conus arteriosus of the right ventricle, the pulmonary trunk courses superiorly and leftward, lying anterior to the ascending aorta and just left of the aortic root when viewed from the left lateral aspect. Near the level of the sternal angle it bifurcates into the right and left pulmonary arteries, with the left pulmonary artery running posterior to the ascending aorta toward the left lung hilum. The left atrial appendage sits anterolateral to the pulmonary trunk, while the coronary sulcus and epicardial coronary vessels contour the ventricular surface inferiorly. Spatially, the pulmonary trunk forms the anterior limb of the great vessel relationship, with the aorta positioned more posterior and medial at its root. This angle matters when you need to teach or plan around the arterial switch operation, where the surgeon must mobilize and transpose the great arteries and reimplant the coronary arteries without kinking the pulmonary outflow tract. It also supports interpretation of congenital outflow malpositions such as transposition of the great arteries, and of acquired dilation of the pulmonary trunk from pulmonary hypertension, where proximity to the aortic root and left atrial appendage affects the operative field and cross-sectional imaging landmarks.
