Description
Arising from the right ventricle at the conus arteriosus (infundibulum), the pulmonary trunk ascends superiorly and slightly posteriorly within the fibrous pericardium, with its root framed by the pericardial reflection around the great vessels. From a right lateral perspective, the vessel lies anterior to the left atrium and left main bronchus and sits left of the ascending aorta, while the right atrium and superior vena cava form a prominent right-sided border nearby. At its distal extent the trunk approaches the level where it divides into the right and left pulmonary arteries, with the right pulmonary artery coursing more horizontally toward the right lung. This right-sided orientation matters because it clarifies the relationships surgeons and imagers depend on when working around the aortopulmonary window and the transverse pericardial sinus, where the pulmonary trunk and ascending aorta define the anterior boundary. Congenital lesions such as pulmonary valve stenosis and tetralogy of Fallot originate at the right ventricular outflow tract and proximal pulmonary trunk, so recognizing the conal anatomy and pericardial investment helps when planning catheter-based intervention or operative repair. Even in adults, post-stenotic dilatation of the pulmonary trunk and pulmonary hypertension alter the contour and position of this segment, changing what you expect on echocardiography, CT angiography, or intraoperative inspection.
