Description

Seen from the left lateral aspect, the left main bronchus gives rise to the left upper lobar (secondary) bronchus, from which the lingular bronchus descends toward the lingula. The inferior lingular segmental bronchus (B5) curves inferiorly and anteriorly as it courses toward the anteroinferior portion of the upper lobe, lying superior to the oblique fissure and anterior to the lower lobe bronchial tree. Proximally it remains medial to the pleural surface, then becomes more anterior as it approaches its segmental distribution. Orientation of the lingular bronchi matters because the lingula is the functional counterpart of the right middle lobe, and its segmental anatomy drives both radiologic localization and bronchoscopic navigation. A characteristic pitfall in bronchoscopy is misidentifying B4 (superior lingular) versus B5 (inferior lingular) when targeting mucus plugging, focal hemorrhage, or a suspected endobronchial lesion, since their takeoff angles differ and the inferior lingular branch often sweeps downward before narrowing distally. Segmental labeling also maps directly to patterns of aspiration, postinfectious atelectasis, and planning for lingula-sparing segmentectomy in early peripheral non small cell lung cancer. Pulmonology and thoracic surgery teaching files can pair this left view with CT bronchial tree reconstructions to reinforce segmental nomenclature and expected branching angles from the left upper lobar bronchus.
