Medially, the tibia descends to the medial malleolus and talus, with the calcaneus forming the posterior heel and the medial longitudinal arch sweeping anteriorly toward the first metatarsal and hallux phalanges. Beneath reflected skin and superficial fascia, the plantar aponeurosis and plantar intrinsic layer are opened to expose flexor hallucis brevis in the medial sole, positioned anterior to the calcaneal tuberosity and deep to the tendinous slips that cross the first metatarsophalangeal region. Posteriorly, gastrocnemius and soleus converge into the calcaneal (Achilles) tendon, providing a clear posterior landmark against the plantar structures. Neurovascular structures track along the medial ankle into the sole. Cleanly. Flexor hallucis brevis matters because it is a key dynamic stabilizer of the first ray and the sesamoid complex at push-off, and its relationship to the first metatarsal head helps explain pain patterns in sesamoiditis and hallux valgus.
