Description
Running along the medial aspect of the knee, the great saphenous vein ascends anterior to the medial femoral epicondyle and continues proximally along the inner thigh, receiving superficial tributaries that form a periarticular venous network. Inferior to the joint line, superficial venules from the medial leg converge toward the saphenous trunk, while deeper channels track posterior to the medial tibial condyle toward the popliteal vein. Perforating veins bridge superficial to deep systems across the medial retinacular and pes anserinus region. Clear topography. Medial knee venous anatomy matters when mapping reflux and planning access, because the saphenous trunk and its tributaries lie close to the saphenous nerve and the medial collateral ligament complex. Varicose vein surgery and endovenous ablation commonly target the great saphenous vein, and iatrogenic nerve irritation is a familiar complication when treatment extends below the knee. This view also helps explain why thrombophlebitis or superficial venous thrombosis near the knee can track along the saphenous pathway and why perforator incompetence around the medial calf may present with localized varicosities.
