Sweeping along the medial hindfoot, the animation isolates the calcaneus and brings the sustentaculum tali into profile as a horizontal, shelf-like projection on the medial surface of the heel bone. Superior to it sits the talus, with the middle facet of the subtalar (talocalcaneal) joint aligned directly above the sustentaculum and the posterior facet curving posteriorly on the calcaneal body. As the viewpoint subtly rotates and settles, the groove on the plantar surface of the sustentaculum for the flexor hallucis longus tendon becomes appreciable as it runs posteroinferior to anteromedial beneath the talus. Orientation stays strictly medial, keeping the sustentaculum anterior to the calcaneal tuberosity and inferior to the talar body. Clinically, this is the landmark that explains a lot of hindfoot pain patterns. Fractures through the calcaneus often spare the sustentaculum as a relatively stable medial fragment (the sustentacular fragment), a key reference in CT-based classification and operative reduction of displaced intra-articular calcaneal fractures.
