Description
Oriented from the plantar surface of the male foot, the medial plantar artery courses anteriorly along the medial border, arising from the posterior tibial artery deep to the flexor retinaculum and entering the sole between abductor hallucis and flexor digitorum brevis. Distally, it runs toward the hallux side of the forefoot, giving superficial and deep branches that track alongside the medial plantar nerve and contribute to the plantar digital supply. Underlying osseous landmarks include the calcaneus posteriorly, talus and navicular more centrally, and the metatarsals and phalanges aligned anteriorly, with the arterial path shown in relation to these bony contours. Adjacent plantar venous channels accompany the arterial route. A plantar perspective is the one you want when teaching or planning around the tarsal tunnel and medial plantar compartment, where the posterior tibial neurovascular bundle divides and becomes vulnerable to entrapment or iatrogenic injury. Surgeons working near the medial calcaneal region, plantar fascia origin, or in approaches to the navicular and first tarsometatarsal joint often use the medial plantar artery as a safety landmark, since its branches can be encountered during releases, tendon procedures, and plantar incisions. It also frames angiosome-based thinking in diabetic foot reconstruction and flap design. Bleeding here is rarely trivial.
