Description
Plantar skin transparency frames the forefoot skeleton while the transverse head of adductor hallucis appears as a slim, horizontal muscular band coursing medial to lateral across the plantar aspect of the metatarsophalangeal region. It lies deep to the plantar aponeurosis and superficial to the metatarsal heads and plantar plates, spanning the distal ends of metatarsals II to V before converging toward the lateral side of the hallux. Medially, its fibers blend with the lateral sesamoid complex and the tendon of flexor hallucis brevis to reach the lateral base of the proximal phalanx of the great toe. Small structure, big consequences. This plantar perspective matters because the transverse head is one of the main dynamic supports of the transverse metatarsal arch, counterbalancing the abductors and influencing hallux alignment during late stance and toe-off. Weakness, denervation, or iatrogenic disruption can contribute to splaying of the metatarsal heads, metatarsalgia, and progression of hallux valgus, while hypertrophy or local scarring can tighten the lateral soft-tissue sleeve around the first metatarsophalangeal joint.
