Description
Running obliquely across the central sole, the oblique head of adductor hallucis arises from the plantar aspects of the cuboid and lateral cuneiform and from slips along the bases of the second to fourth metatarsals. Its muscle belly courses anteromedially, deep to the plantar aponeurosis and long flexor tendons, to join the transverse head near the lateral sesamoid apparatus at the first metatarsophalangeal joint. Medially, the tendon blends with the lateral base of the proximal phalanx of the hallux and the lateral sesamoid complex, lying just lateral to flexor hallucis brevis and inferior to the head of the first metatarsal. Deep to it, the interossei occupy the intermetatarsal spaces, and the deep transverse metatarsal ligament spans the metatarsal heads. A broad midfoot anchor. Functionally, this is the plantar adductor that resists splaying of the forefoot and contributes to stabilization of the first ray during toe-off, a relationship that becomes clinically visible in hallux valgus where the adductor hallucis can pull the proximal phalanx laterally and promote sesamoid subluxation. Surgeons working through medial or dorsal bunion approaches often reference the adductor tendon, lateral capsular structures, and sesamoid attachments when planning a release, and the oblique head’s midfoot origin explains why this tension persists even when only distal soft tissues are addressed. It also frames the corridor used to identify the lateral plantar nerve and artery as they course in the deep plane of the central sole.
