Description
From the plantar (inferior) aspect of the male forefoot, the oblique head of adductor hallucis is isolated in color against the monochrome metatarsals and phalanges. Its fleshy belly arises proximally from the bases of the second to fourth metatarsals and adjacent tarsometatarsal ligaments, then runs anteromedially toward the first metatarsophalangeal joint. Distally, the tendon converges on the lateral side of the hallux, blending with the lateral sesamoid and the lateral base of the proximal phalanx, in close company with the flexor hallucis brevis tendons and the flexor hallucis longus tendon as it passes between the sesamoids. Lateral plantar neurovascular structures typically course deep to this layer. Small but decisive. Teaching the intrinsic plantar muscles becomes easier when the oblique head is separated from the transverse head and from the fibrous plantar aponeurosis. The line of pull clarifies how adductor hallucis counterbalances abductor hallucis at the first ray, stabilizing the sesamoid apparatus during toe-off and resisting lateral drift of the sesamoids as the hallux deviates into valgus. That matters in hallux valgus and bunion surgery, where soft-tissue balancing may include lateral release near the adductor tendon insertion, and in sesamoid pain syndromes where altered mechanics and tendon tension can perpetuate overload beneath the first metatarsal head. Podiatry and orthopedic teaching files often need a clean plantar dissection-style graphic to accompany discussions of hallux valgus deformity, sesamoiditis, and metatarsalgia, and this view pairs well with plantar forefoot MRI or ultrasound correlations of the sesamoid complex. It also fits lower limb anatomy curricula when introducing the lateral plantar nerve distribution and the layered organization of the sole.
