Description
Positioned at the craniovertebral junction, the posterior atlantooccipital joint capsule spans between the occipital bone surrounding the foramen magnum and the superior articular facets of the atlas (C1), with its fibers lying deep to the suboccipital soft tissues. Superiorly, the capsule attaches along the margins of the occipital condyles; inferiorly, it blends with the ring of the atlas near the posterior arch. Just caudal to this articulation, the atlas relates to the axis (C2) at the atlantoaxial joints, and the posterior elements of the upper cervical spine align in the midline beneath the occipital squama. The lambdoid suture frames the posterior calvaria above. Posterior reinforcement at this level matters because it helps define the boundary between physiologic extension at the atlantooccipital joint and pathologic hypermobility after ligamentous injury. Think trauma. In high energy flexion extension mechanisms, capsular disruption can accompany occipitoatlantal instability, a pattern clinicians screen for on CT and MRI when evaluating the craniovertebral junction, and surgeons consider when planning occipitocervical fusion.
