Posterior to the thoracic viscera, the azygos vein ascends along the right anterolateral aspect of the vertebral bodies, receiving posterior intercostal veins segmentally as it climbs through the posterior mediastinum. Medial to it lie the thoracic vertebrae and the vertebral column landmarks, while the hemiazygos vein and accessory hemiazygos occupy a mirrored, more left-sided course before crossing to join the azygos. Superiorly, the arch of the azygos sweeps anteriorly toward the superior vena cava, creating a clear junctional landmark against the posterior thoracic wall. It is a collateral highway. That arrangement matters any time the caval pathway is compromised. In superior vena cava obstruction, the azygos system becomes a high-flow bypass, and its enlargement is a classic imaging clue on chest CT or radiography. Surgeons also care about its posterior mediastinal relationships during right thoracotomy or esophagectomy, where the azygos arch is often divided to improve exposure and the thoracic duct and sympathetic trunks sit nearby and at risk. Educators will find this posterior perspective useful for teaching the azygos-hemiazygos network as the principal drainage route for the posterior thoracic wall and as a connection between the inferior vena cava territory (lumbar veins, ascending lumbar veins) and the superior vena cava.
