Description
Posteriorly, the C6 vertebra is presented with a midline bifid spinous process projecting inferiorly from the junction of the laminae, flanked by the right and left laminae as they arch laterally toward the pedicles. Superior and inferior articular processes sit posterolateral to the vertebral foramen, their facet surfaces oriented to guide flexion and extension while permitting coupled rotation in the lower cervical spine. Lateral to the articular pillars, the transverse processes extend to either side and would incorporate the transverse foramina (foramina transversaria), a defining landmark of cervical vertebrae. The vertebral body lies anterior to these posterior elements, out of direct view in a strict posterior perspective. C6 carries real clinical weight because it anchors surface anatomy and common operative corridors. The anterior tubercle of C6, the carotid tubercle (Chassaignac tubercle), is a palpation point for carotid artery compression and a landmark during anterior cervical discectomy and fusion when confirming level. Posteriorly, the C5 to C6 zygapophysial joints and the C6 spinous process are frequent pain generators after whiplash, and facet-mediated neck pain is often targeted with medial branch blocks and radiofrequency ablation at the corresponding levels.
