Description
Posteriorly, the C4 vertebra is presented with its paired laminae converging in the midline to form the base of the spinous process, which characteristically bifurcates into two inferiorly directed tips. Lateral to the laminae, the superior and inferior articular processes frame the zygapophyseal (facet) joints, while the transverse processes project laterally toward the tips that would carry the anterior and posterior tubercles and enclose the foramen transversarium for the vertebral vessels. Inferior to the spinous process, the vertebral arch outlines the dorsal margin of the vertebral foramen, continuous cranially and caudally with the adjacent cervical levels. Orientation from behind makes the posterior elements easy to read: spinous process, lamina, and facet complex. This is the bony corridor encountered in a midline posterior cervical approach, where exposure proceeds through the nuchal ligament and paraspinal musculature to the spinous processes and laminae before a laminectomy or laminoplasty, and where preservation of the facet joints helps limit postoperative instability. It also supports teaching around typical cervical morphology, including the bifid spinous processes that help distinguish C3 to C6 from C7 (vertebra prominens). Neurosurgery and orthopedic spine courses can pair this view with MRI or CT to connect posterior element anatomy to canal decompression procedures and to patterns of traumatic injury such as laminar fracture or facet dislocation. Medical publishers often need a clean posterior C4 reference when labeling the cervical spine in atlases, exam prep, and patient education on cervical stenosis or radiculopathy.
