Description
Centered in the upper cervical spine, the axis (C2 vertebra) is presented in lateral profile with its thick vertebral body lying anterior to the vertebral arch and a posteriorly projecting, relatively long spinous process. Superiorly, the odontoid process (dens) rises from the body toward the ring of the atlas (C1), creating the osseous pivot for atlanto-axial rotation. The superior and inferior articular processes form the zygapophysial joints with C1 above and C3 below, while the intervertebral disc is seen inferior to C2. The larger vertebral column remains visible behind and below, preserving the cervical lordosis into the thoracic kyphosis and lumbar lordosis. Lateral visualization of C2 matters because this vertebra is the mechanical keystone for head turning, and its dens is a frequent site of clinically significant fracture after falls and motor vehicle trauma. Anderson and D’Alonzo odontoid fractures and traumatic spondylolisthesis of the axis (Hangman fracture through the pars interarticularis) are both assessed in profile on plain radiographs and sagittal CT reconstructions, where anterior translation, angulation, and prevertebral soft-tissue swelling can be correlated with instability. Alignment across the C1 to C2 and C2 to C3 levels is the story here. Small errors in understanding the normal silhouette can lead to missed injuries. Use this illustration in gross anatomy and osteology teaching to differentiate atlas versus axis, to orient the dens relative to the vertebral body and spinous process, and to reinforce cervical spine curvature landmarks.
