Description
Posteriorly, the lower cervical spine is centered on the C7 vertebra (vertebra prominens), with its long, non-bifid spinous process projecting inferiorly in the midline between the laminae. Flanking the spinous process, the left and right superior and inferior articular processes define the zygapophysial joints, while the transverse processes extend laterally toward the costotransverse region of T1. The vertebral arch surrounds the vertebral foramen, continuous superiorly with C6 and inferiorly with the cervicothoracic junction. C7 matters because it is the most reliable palpable landmark in the posterior neck, yet it is also the level where counting errors commonly occur when localizing pathology for injection, surgery, or radiologic reporting. The cervicothoracic junction is a mechanical transition zone, and flexion injury can produce C7 spinous process fractures (clay-shoveler fracture) or facet disruptions that are best understood when the posterior elements and articular pillars are clearly separated. Muscle and ligament attachments concentrate here, including the ligamentum nuchae and the deep cervical extensors inserting onto the spinous processes. Use this artwork in gross anatomy and musculoskeletal courses when teaching cervical vertebrae identification, especially the distinction between bifid upper cervical spinous processes and the prominent C7 spine, and in spine modules that introduce posterior column anatomy and facet orientation.
