Description
Framed from an anterior perspective, the seventh cervical vertebra (C7) is presented at the cervicothoracic junction, with the vertebral body positioned inferior to C6 and immediately superior to T1. The anterior surface of the vertebral body and the lateral margins leading toward the pedicles and transverse processes are emphasized, while the vertebral foramen sits posterior to the body in standard anatomical alignment. Inferiorly, the transition into the upper thoracic vertebrae is apparent, with intervertebral discs interposed between adjacent vertebral bodies. C7 stands out by proportion. Clinically, C7 functions as a reliable landmark when counting vertebral levels and orienting the lower cervical spine, and confusion often arises at the C7 to T1 transition because morphology shifts toward a thoracic pattern. From the anterior aspect, the vertebral body and disc space are the focus for common pathology such as C6 to C7 or C7 to T1 degenerative disc disease, osteophyte formation, and endplate changes that can contribute to cervical radiculopathy or myelopathic symptoms when paired with posterior canal compromise. This is also the level often referenced in anterior cervical discectomy and fusion planning, where accurate level identification and understanding of the cervicothoracic junction’s anatomy reduce wrong-level exposure.
