Description
Arising from the superficial branch of the radial nerve and the dorsal branch of the ulnar nerve, the dorsal digital nerves course distally over the dorsum of the hand, running superficial to the dorsal interossei and extensor tendon expansions and then dividing along the dorsolateral margins of the digits. A posterior (dorsal) perspective places the carpal bones and metacarpals proximally, with the phalanges aligned distal to the metacarpophalangeal joints and the dorsal venous network and dorsal metacarpal vessels tracking in the subcutaneous plane. Sensory branches fan toward the dorsal skin of the thumb, index, middle, and ulnar digits, with predictable overlap near the proximal phalanges. Fine caliber. Clinically exposed. Dorsal digital nerve anatomy matters most when dorsal incisions, lacerations, or percutaneous pin placement threaten cutaneous sensation on the back of the fingers. This view clarifies why injuries near the dorsoradial wrist can produce numbness over the dorsal thumb and index finger (superficial radial nerve neurapraxia, often seen with tight watches, handcuffs, or first dorsal compartment surgery), while ulnar-sided dorsal hand symptoms point you toward the dorsal ulnar cutaneous branch. The relationship to dorsal veins is also practical, since superficial venipuncture and IV placement can irritate adjacent sensory branches and create painful neuromas after seemingly minor trauma. Hand surgery and orthopedic teaching modules can pair this plate with extensor mechanism anatomy to explain safe dorsal approaches to the metacarpals and phalanges, and to map sensory deficits after digital injuries. It also fits cleanly into gross anatomy labs, regional anesthesia instruction (field blocks and wrist blocks), and clinical illustrations for case reports on dorsal hand lacerations or postoperative dysesthesia.
