Arising distally from the common palmar digital nerves, the proper palmar digital nerves run along the medial and lateral margins of each finger, closely apposed to the fibrous digital sheath and deep to the superficial fascia. From a posterior (dorsal) perspective, their course can be appreciated as they curve around the sides of the proximal phalanges toward the palmar surface, lying in proximity to the dorsal digital nerves that supply the dorsal skin over the proximal phalanx and nail complex. Metacarpals, proximal and middle phalanges, and the extensor expansions form the osseotendinous backdrop, with dorsal interossei occupying the intermetacarpal spaces. Small dorsal veins accompany the superficial neurovascular pattern. This angle matters when teaching the split sensory territories of the digits and why dorsal lacerations near the proximal phalanx can still produce paresthesia on the palmar side if the proper palmar digital nerve is tethered or partially transected at the lateral digital margin. It also mirrors the operative corridor for digital nerve repair and neuroma excision, where you often identify the nerve just volar to the mid-axial line and protect the adjacent proper palmar digital artery. Digital blocks and postoperative sensory testing map back to these relationships. Clean anatomy, real consequences.
