Description
Arising in the palm from the median nerve and, on the ulnar side, from the superficial branch of the ulnar nerve, the common palmar digital nerves course distally in the anterior hand toward the web spaces before bifurcating into proper palmar digital nerves for adjacent digits. They travel superficial to the flexor tendons and their synovial sheaths, and run alongside the superficial palmar arterial arch and common palmar digital arteries, forming the familiar neurovascular bundles that enter each interdigital cleft. Lumbrical muscles occupy the deep plane around the metacarpals, while the palmar aponeurosis lies superficially, framing the nerves as they approach the bases of the proximal phalanges. Orientation is straightforward. Proximal forearm structures are included to clarify continuity. Clinically, this anterior projection is the one you want when teaching or planning surgery around the palmar creases and digital web spaces, where the common palmar digital nerves sit at risk during trigger finger release, Dupuytren fasciectomy, and incision and drainage of palmar space infections. Their relationship to the superficial palmar arch explains why lacerations near the distal palmar crease can produce paired findings, brisk bleeding and sharply localized sensory loss along the contiguous sides of two fingers. Variations also matter: a communicating branch between median and ulnar systems (Berrettini anastomosis) can shift sensory deficits after nerve injury and complicate interpretation of nerve conduction studies.
