Description
Arising from the superficial palmar arch, the second common palmar digital artery courses distally within the central palm, aligned with the second intermetacarpal space as it approaches the web space between the index and middle fingers. In an anterior (palmar) view, the vessel is seen superficial to the metacarpals and phalanges, then dividing into proper palmar digital arteries that track along the adjacent sides of the middle two digits. Red arterial channels are paired by blue venous pathways, while the metacarpals, proximal, middle, and distal phalanges provide rigid landmarks for the vascular trajectory. Small transverse anastomoses and terminal branches near the proximal phalanges suggest the dense palmar vascularization that supports grip and fingertip perfusion. Hand surgeons and proceduralists care about this segment because it lies in the operative corridor for palmar lacerations and web space injuries, where bleeding can be brisk and repair requires knowing which common digital vessel has been compromised. The relationship to the digital nerves and flexor tendon sheath is the teaching point: in the palm, the neurovascular bundles run close, so dissection planes during trigger finger release, Dupuytren fasciectomy, or incision and drainage of a palmar space infection must respect the expected course of the common palmar digital arteries.
