Description

Arising from the superficial palmar arch, the third common palmar digital artery courses distally in the mid-palmar soft tissues, aligned with the third intermetacarpal space between the third and fourth metacarpals. It runs anterior to the metacarpals and deep to the palmar aponeurosis, then approaches the interdigital cleft where it divides into proper palmar digital arteries for the adjacent sides of the middle and ring fingers. Nearby, the common palmar digital nerves from the median nerve travel in close company, and the digital veins form a parallel blue network superficial to the arterial trunks. Osseous landmarks include the carpal bones proximally, metacarpal shafts centrally, and the proximal, middle, and distal phalanges distally. Small-caliber branches. Clear orientation. This anterior hand perspective matters when you need to explain why vascular compromise can present with focal ischemic pain or cold intolerance in the third web space, and why blunt or penetrating palmar trauma can disrupt a common palmar digital artery before symptoms appear at the fingertip.
