Description
Arising from the deep palmar arch, the first common palmar digital artery courses distally within the palm toward the first interdigital space, running anterior to the metacarpals and deep to the palmar aponeurosis before dividing into proper palmar digital arteries for adjacent sides of the index and middle digits. Flexor tendons, including the flexor digitorum superficialis and profundus tendons within their fibrous digital sheaths, lie superficially in the central palm, while the lumbricals originate from the FDP tendons on the radial sides and track toward the extensor expansions. The thenar musculature sits laterally, and the interossei occupy the intermetacarpal spaces dorsally and deeply, providing a muscular backdrop to the palmar vasculature. Anterior visualization of this vessel matters because common palmar digital arteries are frequently encountered in volar approaches to the metacarpophalangeal region and in procedures that rely on intact digital perfusion, including replantation, revascularization, and local flap design. Palmar lacerations at the distal palmar crease can transect the artery near its bifurcation while also injuring adjacent common palmar digital nerves and flexor tendons, a combination that explains brisk bleeding paired with sensory loss and impaired finger flexion.
