Description
Centered in the anterior palm, the second common palmar digital artery courses distally from the superficial palmar arch toward the second intermetacarpal space, then bifurcates into proper palmar digital arteries for the adjacent sides of the index and middle fingers. Flexor tendons run longitudinally deep to the palmar aponeurosis toward the phalanges, while the metacarpals and proximal phalanges provide the bony scaffold beneath the vascular plane. Alongside the artery, the corresponding common palmar digital nerve tracks in close company, with superficial veins returning proximally. Small vessel caliber is emphasized. Surgical and procedural anatomy in the central palm often comes down to this relationship: artery and nerve together, tendon deep. During open or endoscopic carpal tunnel release, palmar fasciectomy for Dupuytren disease, or drainage of deep palmar space infection, the second common palmar digital artery sits within the zone where an ill-placed incision can injure paired neurovascular bundles and compromise digital perfusion. This anterior perspective also supports teaching of collateral flow, because the common palmar digital arteries represent the primary channels distributing blood from the superficial palmar arch to the proper digital vessels, with variable contributions from the deep palmar system. Hand surgery texts, anatomy lab practicals, and vascular teaching modules benefit from a clean depiction of the second common palmar digital artery dividing toward the index and middle digits, including its proximity to the common palmar digital nerve and the flexor tendon sheath.
