Description
Posterior to the radius and ulna, the superficial extensor compartment of the forearm is arranged in broad longitudinal bands that continue distally as tendons across the dorsum of the wrist and into the hand. From the lateral (radial) side, brachioradialis frames the extensor mass, with extensor carpi radialis longus and brevis lying just medial to it, while extensor digitorum occupies the central posterior compartment with extensor digiti minimi and extensor carpi ulnaris positioned progressively more ulnar. Proximally, the lateral elbow region blends toward the supinator deep to the radial-sided extensors, and distally the extensor tendons align toward the metacarpals and dorsal digital expansions. Clear tendon paths. That matters. A posterior perspective of these structures is the working view for localizing pathology at the lateral epicondyle and along the extensor tendons. Lateral epicondylitis most often involves the common extensor origin and extensor carpi radialis brevis, and this angle clarifies why pain provoked by resisted wrist extension or gripping maps to the radial-side extensor group rather than the flexor compartment. It also supports teaching of radial nerve anatomy in context, since the posterior interosseous nerve traverses the supinator (Arcade of Frohse) and can be compressed with finger extension weakness while sparing wrist extension.
