Description
Anterior views of both crura are rendered from the level of the knee to the toes, with the tibia medial to the fibula and the anterior compartment muscles layered superficial to the interosseous membrane. Tibialis anterior occupies the anterolateral surface of the tibia, while extensor hallucis longus runs more centrally and extensor digitorum longus lies lateral, approaching the fibula proximally. Distally, the extensor tendons pass deep to the extensor retinacula across the anterior ankle and fan onto the dorsum of the foot toward the hallux and digits II to V. Bone detail includes the tibia, fibula, tarsals, metatarsals, and phalanges, with the patella and periarticular knee ligaments indicated superiorly. This anterior compartment view matters because it ties dorsiflexion mechanics directly to palpable and clinically tested structures. Weakness of tibialis anterior and extensor hallucis longus is a practical clue in common fibular (peroneal) nerve palsy at the fibular neck, and the course of these muscles clarifies the resulting foot drop and high-stepping gait. The anterior ankle also serves as a landmark region for anterior compartment syndrome and for planning incisions and avoiding tendon injury during approaches to the distal tibia and ankle.
