Description
Running along the anterolateral shank, the tibia sits medial and subcutaneous with its sharp anterior border, while the fibula lies lateral, slimmer, and partly obscured by overlying muscle bellies. Tibialis anterior occupies the most medial position of the anterior compartment, with extensor hallucis longus and extensor digitorum longus coursing more laterally toward the dorsum of the foot and the extensor tendons heading distally under the extensor retinacula at the ankle. Bony landmarks at both ends frame the compartment, including the patella and proximal tibia superiorly, and the distal tibia and fibula forming the ankle mortise around the talus inferiorly. Posterior calf contour from the gastrocnemius may still be appreciated at the margins, even from an anterior perspective. Anterior compartment anatomy matters because it concentrates structures commonly implicated in exertional anterior compartment syndrome, where rising intracompartmental pressure compromises perfusion and produces pain with dorsiflexion weakness. Injury to the common fibular (peroneal) nerve at the fibular neck, or entrapment of the deep fibular nerve beneath extensor hallucis longus, classically presents with foot drop and sensory change in the first dorsal web space. These relationships also ground everyday diagnoses, from tibialis anterior tendinopathy and medial tibial stress syndrome to anterior ankle impingement where extensor tendons and retinacula become symptomatic. Use this anterior lower leg muscle plate in gross anatomy and kinesiology teaching to orient students to compartment boundaries, tendon trajectories, and palpation landmarks from the tibial crest to the dorsum of the foot.
