Description
Posterior bony landmarks of the male pelvis are rendered with the lower lumbar vertebrae superiorly, the sacrum and coccyx in the midline, and the paired hip bones flanking them laterally. The left ilium carries a highlighted fracture line with displacement along the superior margin of the iliac crest, lying lateral to the posterior superior iliac spine and superior to the greater sciatic notch. Inferiorly, the proximal femora align with the acetabula, establishing the relationship between the iliac wing (ala), the sacroiliac region, and the hip joints. Iliac wing fractures often occur after high-energy blunt trauma and are frequently discussed as part of pelvic ring injury patterns, even when the acetabulum remains intact. Posterior orientation matters because the iliac crest and posterior superior iliac spine are palpable landmarks used during assessment and during surgical exposure, and crest displacement can signal associated sacroiliac ligament disruption or occult extension toward the sacroiliac joint. Alignment at the sacroiliac joints and the superior iliac crest also helps explain why pelvic binders, external fixation, or posterior pelvic fixation decisions hinge on stability rather than the fracture line alone.
