Description
Prominently framed from an anterior perspective, the male pelvic girdle is shown with the right ilium disrupted by an irregular fracture traversing the iliac fossa, with the fracture zone emphasized in red. The iliac crest forms the superior border, while the anterior superior and anterior inferior iliac spines sit anterolaterally above the acetabulum. Inferomedially, the pubis and ischium complete the acetabular ring, and both femoral heads are seated in their respective acetabula; the sacrum and lower lumbar vertebrae with intervertebral discs appear centrally, posterior to the pelvic brim. Anterior iliac fractures matter because they sit at the interface between bony stability and abdominal and flank anatomy, and they often signal higher-energy pelvic ring trauma even when the acetabulum and hip joint space look preserved. A fracture through the iliac wing can accompany iliacus muscle hematoma and irritation of the femoral nerve as it courses on the iliacus, producing anterior thigh pain or quadriceps weakness that can be missed when attention stays on the hip. Surgical planning also depends on recognizing whether the fracture approaches the pelvic brim or acetabular columns, since fixation strategies differ for an isolated iliac wing injury versus a true ring disruption.
