Description
Prominent in this anterior lower-torso view are the right and left hip bones with the ilium forming the superior lateral walls of the pelvis, the sacrum centered posteriorly, and the lumbar vertebrae descending to the lumbosacral junction. On the right os coxae, a fracture line is emphasized at the anterior inferior iliac spine (AIIS), just inferior to the anterior superior iliac spine and anteroinferior to the iliac crest, approaching the supra-acetabular ilium. Inferiorly, the pubic bodies and obturator foramina frame the anterior pelvic ring, while the femoral heads sit laterally, articulating with the acetabula to define both hip joints. Soft tissues are rendered semi-transparent to keep bony landmarks readable. Centering the trauma on the AIIS is clinically specific because this bony prominence anchors the direct head of rectus femoris and lies adjacent to the anterosuperior acetabular rim, so fractures here can represent either high-energy pelvic trauma or an avulsion pattern in younger, athletic patients. Misinterpreting an AIIS fragment as a simple iliac wing break can change management, since displacement may alter hip flexor mechanics and, when prominent, contribute to extra-articular anterior hip impingement and pain with flexion. Reduction planning also depends on appreciating the relationship to the acetabulum and to nearby neurovascular structures traveling through the pelvis. Small landmark. Big implications.
