Description
Posterior pelvic anatomy is presented with the sacrum centered between the iliac bones and the lower lumbar vertebrae superiorly, while the proximal femur and hip joint sit laterally. The lateral sacral artery descends on the posterior surface of the sacrum, tracking medial to the posterior iliac region and parallel to the row of posterior sacral foramina, where segmental branches enter toward the sacral canal. Paired venous channels accompany the arterial course, and fine neural elements align near the foraminal margins in keeping with the dorsal rami and sacral nerve root exits. A posterior perspective on the lateral sacral artery matters because it clarifies the vessel’s relationship to the posterior sacral foramina and dorsal sacral nerve pathways, a common blind spot in learners who only encounter the internal iliac system from an anterior approach. Bleeding from sacral and pelvic fractures, posterior approaches to sacroiliac fixation, and placement of iliosacral screws all benefit from an accurate mental map of where this artery runs on the dorsal sacrum and where its branches disappear through the foramina. Small caliber does not mean low consequence. Sacral foraminal branches can contribute to hematoma adjacent to the sacral canal and complicate exposure in posterior pelvic surgery.
