Description
Posterior pelvic anatomy is arranged around the sacrum and coccyx, with the ala of the sacrum flanked by the posterior ilium and the sacroiliac joints. Running along the lateral sacral foramina, the lateral sacral vein courses inferiorly and medially on the dorsal surface of the sacrum, receiving small venules from the sacral canal region before turning laterally toward the pelvic sidewall. Near the greater sciatic notch it joins the internal iliac vein, where accompanying internal iliac vessels and elements of the sacral plexus sit anterolateral to the sacrum in close neighborhood. For pelvic surgeons and interventionalists, this venous channel matters because it forms part of the low pressure, valveless venous pathways that communicate with the internal vertebral venous plexus and presacral venous plexus. Bleeding here is different. During posterior pelvic fixation, sacroiliac joint approaches, or presacral dissection (for example, low anterior resection or mobilization near the promontory), traction or instrument placement near the sacral foramina can tear lateral sacral tributaries, and venous back-bleeding may persist because of pelvic venous interconnections rather than a single proximal control point. It is also a useful landmark when teaching the venous drainage pattern of the sacrum in relation to the internal iliac vein.
