Posterior Perspective of the Middle Colic Artery

Description

Arising from the superior mesenteric artery, the middle colic artery courses superiorly and anteriorly into the root of the transverse mesocolon, where it divides into right and left branches that run along the inferior border of the transverse colon. From a posterior perspective, the transverse mesocolon is read as a vascular sheet draped in front of the pancreas and third part of the duodenum, with the middle colic vessels positioned centrally between the right colic and left colic territories. Marginal arterial continuity along the colon can be inferred where the middle colic anastomoses laterally with the right colic near the hepatic flexure and with the left colic (via the inferior mesenteric system) toward the splenic flexure. Venous counterparts, including the middle colic vein draining toward the superior mesenteric vein, accompany the arterial branches. This posterior angle matters when you are thinking like an abdominal surgeon. Middle colic ligation during extended right hemicolectomy or transverse colectomy hinges on identifying the vessel at the mesocolic root while protecting the superior mesenteric vessels and the gastrocolic trunk of Henle, and this is also where an accessory right colic artery, if present, can change the safe dissection plane. Ischemia at the splenic flexure (Griffiths point) becomes easier to explain when you can trace how the middle colic contribution meets, or fails to meet, the left colic supply through the marginal artery.
